As an employer offering ERISA compliant benefit plans, Triad is required to provide notices and reports to employees throughout the course of the year. Generally, the reports are informational only, and no action is required on the part of the employee.

IMPORTANT HEALTH COVERAGE TAX DOCUMENTS (Form 1095C)
Change to Distribution of Form 1095-C for Non-Full-Time Employees and Non-employees
The IRS has provided guidance for self-funded medical insurers, giving us a reprieve from mailing out 1095-C forms to anyone that was not a full-time employee any month during the calendar year. You can request to receive a copy of your 1095-C form by sending an email to benefits@lanl.gov or mail a written request to: Triad National Security, LLC, Benefits Office, PO Box 1663, MS P280, Los Alamos, NM, 87545. If you have any questions, please contact Benefits Office at 505-664-MYHR (4-6947), option 3.

Distribution of 1095-C to Full-Time Employees
Triad will furnish the Form 1095-C to all full-time employees by mailing the forms to the employees’ mailing address for each plan year before March 2 of the following year. For purposes of Form 1095-C, full-time employees are employees who worked 130 or more hours for at least one month of the plan year.

Annual reports for the following plans are mailed to corresponding participants for informational and compliance purposes. The summaries provide an overview of each plan's financial activity from January 1 through December 31 as required under ERISA.
